Transitional Discharge Model

The Transitional Discharge Model (TDM) is a person-centered framework designed to support individuals with mental health and/or addiction challenges to move from hospital care back to community living.

The key benefits of TDM:

Based on relationships: built on the assumption that individuals recover best through continued, trusted relationships.

Improved outcomes: TDM functions to reduce hospital readmission rates, reduce length of stays and lowers overall health care costs.

Support for transition: Addresses the high-risk, vulnerable period immediately following discharge by providing consistent, person-centred support.
